To assist the school in planning the upcoming school year, please register your children as early as possible.
To register for the 2024-2025 school year, children must be 5 years old on or before December 31, 2024.
Registration forms are available from the school office.
Both the Registration and Medical Questionnaire must be completed for all new registrations. The Medical Questionnaire is required even if there are no issues. If there is a medical concern please identify it on the Medical Questionnaire so the school is aware.
If applicable, please attach a copy of the most recent report card.
As outlined by the Department of Records and LSSD, the school requires the following documents:
- A copy of the child's Birth Certificate. (Birth certificates can be obtained from: Vital Statistics Agency 254 Portage Avenue Winnipeg, MB R3C 0B6 1-800-282-8069 ext.3701)
- Proof of Citizenship and parent/guardian proof of residency must accompany all registrations. Proof of residency documents accepted: Driver's License, Lease Agreement, Mortgage, Utility Bill. (Manitoba Health is not accepted as proof of residency)
To avoid delays in processing, please forward all required documents with the registration.
Transportation Requests: Bussing is provided to all eligible students in the Lord Selkirk School Division. If bussing is required, please complete the bus transportation form. If a bus is not required, please indicate your intent on the registration form.
School of Choice forms: should be completed ONLY if applying to attend a school out of the home school catchment area.
Child in Care forms: should be completed ONLY if child is in the care of CFS.
It is the responsibility of the parent/guardian to supply all the necessary documentation for school registrations. This checklist should help you along:
